Documented disabilities may be psychological, neurological, medical, learning, hearing loss or Deaf, vision loss or Blind, or mobility issues. In order to request academic, housing, and/or dietary accommodations, students must submit an application which includes their student testimony. Students must also attach supportive documentation to the application or have their provider fax documentation to our office. Accessibility Services will then review your accommodation requests and respond with a decision.

STEP 2: Submit Supporting Documentation
Qualifying documentation includes verification of a student’s disability or diagnosis which can include a neuropsychological, psychoeducational, and/or psychological evaluation. If those do not apply, then documentation from a licensed practitioner which verifies and details the student's disability or diagnosis with the forms listed below. This information is needed so we can determine appropriate accommodations based on how your diagnosis impacts a major life activity including but not limited to learning, walking, hearing, seeing, reading, and speaking. Please note that any documentation submitted must be completed by a qualified professional (i.e. medical doctor, psychologist, counselor, etc.). Individualized Education Plans and 504 Plans from highschool are accepted, however need to be submitted in conjunction with an evaluation or verification form listed below.

STEP 3. Intake Meeting
Once your application and documentation have been received, you will be emailed to set up an intake meeting with a member of our staff. This interactive process allows our office to determine the barriers of the individual student’s access to their education.
STEP 4: Accommodation Letter
You will receive an accommodation letter detailing your approved accommodations. These accommodations are approved for the entirety of your time at Endicott, with the exception of renewing your housing accommodation each year and any parking waivers that have been approved for one semester.
Your professors will also receive your accommodation letter in the fall and spring semesters to ensure that your accommodations are implemented.
